Documenting Service Hours

All candidates for the National Honor Society must provide written verification of their service hours. Please see the Criteria for Selection section for details on acceptable service. This verification will be maintained by the student throughout high school and attached to the NHS application packet their junior year.
Verification can include: certificates, letters (on organization letterhead), service hours log sheets (must include the name of the organization and a signature) or emails sent to the student from an official organization email (ie not a personal email account) that includes the necessary information.
Documentation must include:
Name of Organization
Hours served (with dates)
Type of service performed
Supervisor name and an email address at which to contact them for verification
Supervisor signature
If the organization does not provide certificates or letters documenting service hours, students can use the Service Hours Form.
Students must document all submitted hours for verification by the NHS selection committee.
