8th Grade DC Trip: Medical Forms Due
All parents of 8th graders who are going to Washington DC should read the letter from Nurse Angie Lucas, and download the medication form if necessary. Any student who will carry over-the-counter or prescription medication, or who should have teachers carrying prescription medication for them on the trip MUST have the form filled out for each medication. All forms and medication to be carried by the teachers on the trip must be reviewed by Mrs. Lucas on or before April 21st.
